ID REQUIREMENTS

California State Law requires practitioners to seek proof of age and relation beyond a reasonable doubt when working with minors. Requirements are as follows:

UNACCEPTABLE FORMS OF ID

 

 

  • School ID (even with a birth certificate)

  • Birth Certificate

  • DMV Permit/Temp License

  • Social Security Card**

  • Photograph of ID

  • A parent, sibling, or other non guardian adult signing for an adult child over 18

​

**SSA recommends to leave your social security card at home in a safe place. It is not an ID.

18+ (ADULT) VALID FORMS OF ID

 

  • Driver's License

  • State ID

  • Passport (US or Foreign)

  • Passport ID Card

  • Military ID*

*MUST have your date of birth printed on it to be a valid ID. We prefer forms of ID that we can photocopy as Military ID's cannot be copied, but a military ID is still acceptable to show proof of age.

 

ID provided also MUST be non-expired, and current.

​FOR MINORS UNDER THE AGE OF 18

I will pierce age 6 and up with parent/guardian consent, presence, and proper identifications of both parent/guardian and child.

You will need your natural parent, or legal guardian/adoptive parent to bring you - NOT A SIBLING (unless they are a LEGAL, COURT ORDAINED GUARDIAN, WITH PROOF) - and be present during the entire procedure.

 

You will also require some documents as proof of relationship to submit on your contract to the studio that may be audited by the health department.

  1. A natural parent or legal guardian* MUST be present. Only one parent is needed. THEY MUST STAY FOR THE ENTIRE PROCEDURE WITHIN THE BUILDING.

  2. I require ONE form of ID listed for adults for the parent.

  3. A school ID for the child, and/or an ORIGINAL or CERTIFIED COPY of their birth certificate.

  4. Notarized paperwork IN ADDITION TO the above indicating legal guardianship over minor in the event you are a step parent, adoptive parent, or other non-natural guardian.


*Legal guardian meaning legally ordained through the courts. Verbal orders are not sufficient. Illegally signing for a minor could lead to legal repercussions for the signing party.


BEING OVER THE AGE OF 18 DOES NOT MAKE YOU A LEGAL GUARDIAN OR GIVE ANY LEGAL ABILITY TO SIGN A CONTRACT FOR A MINOR. PARENT OR LEGAL GUARDIAN ONLY. NO EXCEPTIONS.

AGE REQUIREMENTS


The youngest I will pierce is 6 years old and up, given they are ready for the procedure. Here is a guideline for procedures I will do for what ages.

*Will not perform any lip piercing if braces are present.

*Subject to evaluation. If I feel they are not ready for the procedure they ask for, I will decline the service.

SERVICE REQUIREMENTS

VACCINE CARDS

At this time, we DO NOT require to see or copy vaccination cards to perform anything under the mask. We will continue to comply with county, city, and state guidelines, if necessary we may change this policy to reflect their recommendations and laws.

ILLNESS

If you do not feel well, or have not felt well in the days leading up to seeing me, I urge you to stay home and return another day when you are feeling better.

If you have been in contact with anyone diagnosed with COVID-19, we also ask you to refrain from visiting until you are either declared negative or if COVID positive, after 14 days from that date.

RIGHT TO REFUSE SERVICE

We retain the right to refuse service for any reason.

If we have reasonable belief that you are ill, have been in contact with someone who is ill, or show any symptoms that may make us believe you are not in a condition to proceed with a procedure.

If we feel we cannot adequately serve your needs, we will also refuse service.

TOOL & NEEDLE STERILZATION

i personally guarantee all tools used in your procedures have been processed (sterilized) and packaged for a single use. All needles are single use, disposable ones. I inspect them before use to ensure no damage or spurs to their finish.

I do not open any tools until you are in the room with me to visibly see for your procedure, this way you can see that all items used are sterile for you.
